USA Hockey Consent to Treat/Medical Form
All players under the age of 18 are required to have a parent or legal guardian complete and sign the USA Hockey Consent to Treat / Medical Form. Players who are 18 years of age or older must sign the form themselves.
This signed authorization allows on-site medical personnel to evaluate and administer medical care if it becomes necessary. For validity and compliance, the form must be physically signed in ink (electronic or typed signatures are not accepted).
